This is the text of a letter that we sent out to the bishoprics and clerks 
in my stake.  Perhaps this might be helpful.

"Re: New Bar Coded Temple Recommend Books

Bishops,

You should have received your new recommend books along with instructions. 
Please have your bishopric and clerks review the material and this letter 
carefully.  Beginning July 2, 2007, do not issue any recommends using the 
old forms.

In issuing the new bar-coded recommends, there will be two types of 
processes.  The first is the conversion to the new bar coded recommends for 
those that have a current old-style recommend.  This type requires no 
interview and reuses the existing expiration date.  The second process is 
the standard type for members who require a renewal or reevaluation and 
therefore will need an interview.

As a stake, beginning on Sunday July 15, 2007, we will start the process of 
converting to the new recommends.  Using the attached schedule, a member of 
the stake presidency will be available at your building for an hour to sign 
recommends for whomever you can have ready.  (We would like a minimum of 25)

Individuals requiring an interview with a member of the stake presidency 
will not be accommodated on this schedule.  For recommends requiring an 
interview, have the member using the standard renewal process, contact the 
stake executive secretary for an appointment.

For a successful completion of this conversion process, instruct the member 
that they will need to present their old style valid recommend to the 
bishopric who will fill out and sign the new bar coded recommend in their 
presence.  After that, the member will present the old recommend to the 
stake presidency along with the new bar coded white original that has the 
attached canary copy.  The stake presidency will keep the old recommend and 
the canary copy of the new one.  Any member who does not present a valid old 
recommend must go through the normal interview process to have a new one 
signed.  Please do not have the bishopric member collect or destroy the old 
style valid recommend in this conversion process.

For members whose old style recommends expire before the end of this year, 
consider having them renew their recommend through the normal recommend 
interview process, as it will reset the two-year expiration period.

When copying data, please recheck member record numbers against a current 
list from MLS.

We have 120 days to complete the changeover before the temple no longer 
accepts the old non-bar coded recommends.

Until we learn how fast the system works, please advise members that it may 
take up to 48 hours for the bar coded recommend to be activated and temple 
ready after it is signed by the stake presidency.

By the end of October, make sure your clerks have delivered all the old 
recommend books to the stake office for disposal.  We have to log in the 
books, record the unused numbers and report them to headquarters.  Please do 
not have your clerk's dispose of them themselves.
