Curious, indeed. It's almost as if the clerk form is "bootlegged." :-) But
the form does say if there are any questions, you should call the much-loved
extension 2-3500, so I don't know why someone at Local Unit Support wouldn't
know. However, the Stake Information Change Request, which deals with the
stake presidency and patriarch(s), does have a number: 35633. Perhaps that
would jog someone's memory if you were to call back.
Another (better?) option: call the main number and ask for Church
Organization Services, which is the office that collects the forms.
